Traffic lights at Crous Drive and Westgate Road near Westgate shopping center were repaired earlier this week after three months of not working.
The Johannesburg Road Agency sent a technician to the site on Wednesday after being alerted by the record.
The intersection is busiest during morning and afternoon peak hours, which led to congested traffic.
Just this week, traffic lights have been off at Golf Club Terrace since Tuesday, while Wednesday morning the traffic lights at Hendrik Potgieter Road caused cars to queue heading east.
